Nursing Home Transition

As a Center for Independent Living, Disability Options Network makes it a priority to support individuals who wish to leave a nursing home and return to the community. This service assists people with disabilities to find accessible housing, transportation, personal assistance services, and other community resources necessary for full inclusion into society. We also strive to divert people from going into a nursing home by supporting them through the Medicaid Waiver enrollment process so they can access in-home, non-medical personal assistant services for help with daily activities.  The goal is to provide an alternative to living in a facility and give consumers the choice to age with the services they need to remain at home.

The CIL staff can assist with the process through the Independent Enrollment Broker, selected by the state, to guide people through each step of the process. There can be a great deal of paperwork and it is often confusing, that is why our staff is here to assist with the process.
